Primary Care Networks and CHCs: Webinar

BCACHC was pleased to partner with the Ministry of Health and Doctors of BC to host a webinar on Community Health Centres and Primary Care Networks (PCNs). PCNs are the key mechanism by which team-based care is delivered in BC, linking a mix of service designs, including the CHC model. Through this model, primary care is designed locally and collaboratively based on needs and service gaps.

Collaborative models succeed when everyone at the table is up to speed on the roles, goals, and opportunities brought forward by the other members of the team. This webinar provided a great chance for learning about the Community Health Centre model and its role in team-based care delivery in BC.
